The goal: Transform monitoring from data collection exercise to decision support system. The Question You Must Answer Your facility has monitoring equipment. That’s not in question. But ask yourself: If a problem is developing - gradually, subtly, over weeks or months - are you confident your monitoring system would detect it early enough to intervene? Not: “Do we have instruments that could theoretically detect it?” But: “Would our system, as actually implemented, with real humans operating it, catch it?” Follow-up questions:
Could the signal be lost in noise? (Too much data, no prioritization) Could it fall between review cycles? (Problem develops between monthly reviews) Could it be masked by normal variation? (Gradual trend hidden in seasonal fluctuations) Could it be dismissed as unimportant? (No clear threshold for action) Could it be visible in data but not noticed by anyone? (No one actually analyzing the data effectively) Could it be noticed but not escalated? (Unclear responsibilities or fear of raising false alarms) Could it be escalated but not acted upon quickly? (Bureaucratic response processes)
If you answered “yes” or “maybe” to any of these, your monitoring system has vulnerabilities. Flight 447 had perfect data collection. The crew still couldn’t save the aircraft because they didn’t understand what the data was telling them. Your tailings facility might have perfect instrumentation. But if the monitoring system - people, processes, analysis, decision-making - isn’t working effectively, the data won’t save anyone either. The black box only matters if someone understands what it’s recording, analyzes it appropriately, and acts on what it reveals. Is your monitoring system designed to save lives? Or just to document what happened? The difference is everything.
